Care Team Assistant - Job Overview

Schedule: Monday-Friday (8AM-5PM)

Key Responsibilities

  • Patient Communication: Be the voice of support! Address patient calls, communicate lab results, and ensure patients feel informed and cared for.
  • Pre-Visit Planning: Channel your inner detective to review schedules and gather essential information in the electronic health record system (EHRS).
  • Care Coordination: Guide patients through transitions of care and ensure they have access to all the services they need with your stellar coordination skills!
  • Medication Management: Assist in smooth sailing medication refills and handle prior-authorizations like a pro!
  • Forms and Referrals: Get those forms ready and assist with referrals to keep everything moving smoothly.
  • Safety and Compliance: Be a safety champion! Participate in training and maintain a safe environment for all.

Minimum Qualifications

  • Education: A high-school diploma or equivalent is your ticket in!
  • Certifications: Got a Medical Assistant Certification or at least 2 years of clinical experience? Awesome! And make sure that BLS/CPR certification is shining bright.
  • Experience: Two years of experience in an ambulatory clinical setting would be fantastic!
  • Skills: Acing the EHRS, excellent communication, multitasking like a ninja, fabulous organizational skills, and typing/computer wizardry.
  • Extra Points: Fluent in Spanish? That would be a huge plus!
  • Discretion: Handle confidential info like the secret keeper you are!
