Care Team Assistant - Job Overview
Schedule: Monday-Friday (8AM-5PM)

Key Responsibilities
- Patient Communication: Be the tech-savvy voice that supports patients, ensuring they receive the information they need with digital empathy and care.
- Pre-Visit Planning: Collaborate with our agile team to review schedules and gather essential information from electronic health record systems (EHRS) for cutting-edge patient visit preparation.
- Care Coordination: Assist in orchestrating patient care to ensure they have streamlined access to all necessary healthcare services.
- Medication Management: Manage medication refills and prior-authorizations with precision and data-driven accuracy.
- Forms and Referrals: Digitally facilitate the preparation of forms and streamline the referral process to optimize patient care pathways.
- Safety and Compliance: Contribute to a secure and compliant digital environment through continuous learning and adherence to cutting-edge safety protocols.
Minimum Qualifications
- Education: A high-school diploma or equivalent is the launchpad for your journey into healthcare innovation.
- Certifications: A Medical Assistant Certification or minimum two years of clinical experience is required, along with a current BLS/CPR certification.
- Experience: At least two years in an ambulatory clinical setting, where you've embraced tech-driven healthcare solutions.
- Skills: Mastery of EHRS, superior communication, tech-savvy multitasking, and organizational prowess are essential.
- Extra Points: Bilingual proficiency in Spanish is a game-changing asset!
- Discretion: Handle confidential information with the utmost integrity and advanced data protection practices.
