Lead Perinatal Care Navigator - Job Overview

Key Responsibilities

  • Revolutionize perinatal navigation support and deliver comprehensive care coordination for OB and pediatric patients.
  • Mentor and train the Perinatal Navigator and Perinatal Care Coordinator staff, ensuring high-quality navigation services through visionary leadership.
  • Disrupt traditional barriers by facilitating seamless communication across departments, promoting integrated services.
  • Organize and lead innovative meetings for Patient Education/Perinatal Navigation programs, managing audit tools and driving program evaluation.
  • Develop and oversee cutting-edge systems for tracking care coordination activities, patient progress, and navigation workflows/outcomes.
  • Contribute to program development and evaluation, championing quality improvement initiatives and strategic planning for Legacy OB and Pediatric patients.
  • Implement forward-thinking health education programming to empower patients in managing health conditions and enhancing wellness.
  • Provide coverage for critical point-of-care services, including health screenings and risk assessments.
  • Guide patients in navigating the care system, helping them achieve their health goals.
  • Document all patient services meticulously in electronic health records and other data systems.
  • Support and nurture relationships with hospital partners, coordinating care for Legacy newborn and postpartum patients.
  • Serve as a pivotal liaison between clients, healthcare providers, and community agencies.
  • Lead or contribute to outreach events, workshops, and community engagement efforts, driving community impact.

Minimum Qualifications

  • Bachelor's degree or equivalent in a related field.
  • Minimum of three (3) years of leadership experience in managing practice operations or programs, including supervising direct reports.
  • Alternatively, two (2) years in a relevant internal position with progressively responsible duties, preferably with supervisory experience.
  • Proficient in decision-making responsibilities, with a penchant for innovative solutions.
  • Strong communication skills, both oral and written, to articulate complex ideas clearly.
  • Foundational understanding of statistics and graphical representation principles for data-driven decision making.
  • Technologically adept, with proficiency in electronic mail, word processing, and office management software.
  • Ability to manage multiple deadlines and prioritize tasks effectively in a dynamic environment.
  • Strategic decision-making skills and ability to navigate multiple deadlines.
