RN - Care Manager (Job Overview)

Schedule: Monday-Friday (8AM-5PM)

Key Responsibilities

  • Collaborate with a multidisciplinary team to address the unmet needs of patients.
  • Conduct telephonic outreach and home/virtual visits based on patient acuity.
  • Perform comprehensive health risk assessments and disease management evaluations.
  • Provide outreach, education, and interventions for patients with high utilization patterns.
  • Develop patient-centered care plans in collaboration with patients and care teams.
  • Coordinate services to optimize clinical outcomes and track patient progress.
  • Identify and address barriers to treatment, facilitating resources and referrals.
  • Build collaborative patient relationships through education on health preservation and self-management.
  • Participate in regular caseload reviews and consultations with the care team.
  • Assist in securing medical supplies for patients as needed.
  • Maintain meticulous medical documentation and adhere to care management standards.
  • Contribute to the development of new care standards and participate in staff meetings and educational offerings.

Minimum Qualifications

• Registered Nurse with an active, unrestricted Texas licensure. • 3-5 years RN experience in an acute care setting; other clinical settings may be considered. • 1-2 years case/care management experience. • Experience with all patient populations, including Commercial, Medicaid & Medicare. • Computer literacy with ability to operate computer programs and documentation platforms. • Strong customer service skills for service delivery and resolution of issues. • Discretion and secure information management. • Desirable: Case Management Certification, Motivational interviewing, Federally Qualified Health Clinic (FQHC) experience, Bilingual (Spanish), Ability to think critically and problem solve.
