Key Responsibilities

  • Unleash health education magic in one-on-one and group settings. Captivate your audience!
  • Employ motivational interviewing and goal-setting wizardry to tailor care to patient needs.
  • Be the ultimate connector with referrals and service linkages to empower patient independence!
  • Swing into action with point-of-care testing for HIV and other conditions.
  • Spark innovation by contributing to new health education program development.
  • Collaborate with a super squad of interdisciplinary clinic and care team members!
  • Assess patient needs like a detective and track their heroic progress!
  • Forge referral processes and build unstoppable community networks!
  • Champion culturally sensitive care that’s timely and tailored!
  • Record patient services with the precision of a ninja in our electronic health record.

Minimum Qualifications

  • High School Diploma or equivalent? Check! You’re off to a great start!
  • 3-5 years of relevant experience in a related field? You’ve got the experience chops we need!
  • Bachelor’s Degree in a related field with at least 1 year of relevant experience? That's what we love to see!
  • Experience in performing point-of-care testing and educating diverse populations is your secret weapon!
  • HIV care and prevention competencies? You can master them within 3 months of hire like a pro!
  • Phlebotomy training and bilingual proficiency in English/Spanish? Consider it a bonus level up!
  • Proficiency in Word, Excel, and other computer programs? You're the tech-savvy superstar we're looking for!
