Care Team Assistant - LSC (Job Overview)
Schedule: Monday-Friday (8AM-5PM)

Key Responsibilities
- Serve as the frontline responder for patient communication, ensuring effective coordination with the care team.
- Facilitate comprehensive pre-visit preparations to enhance patient experience.
- Support seamless transitions of care, ensuring continuity and quality of service.
- Manage the Care Coordination Desktop, keeping patient information organized and accessible.
- Assist in medication management, ensuring patients have the resources they need.
- Initiate the completion of critical forms to support patient care plans.
- Collaborate on referrals, connecting patients with necessary community resources.
Minimum Qualifications
- High School Diploma or equivalent.
- Medical Assistant Certification or equivalent experience preferred.
- Alternatively, 2 years of relevant experience in an inpatient or outpatient setting.
- 2 years of experience in an ambulatory clinical setting is preferred.
- Bilingual in English and Spanish is preferred, enhancing communication with a diverse patient population.
