Key Responsibilities
- Deliver pharmaceutical care services through direct, innovative interactions with patients and healthcare providers.
- Conduct both face-to-face and virtual consultations to manage chronic disease states with cutting-edge medication management.
- Utilize advanced techniques to obtain and analyze comprehensive medication histories for optimized patient care.
- Reconcile patient medications during ambulatory care visits, ensuring safety and efficacy.
- Engage in physical assessments and patient triaging as necessary, employing the latest diagnostic tools.
- Document encounters and interventions meticulously within an electronic medical record system.
- Design, implement, and assess therapeutic plans, utilizing state-of-the-art, evidence-based practices.
- Manage medication therapy, including initiation and modification, within approved procedural guidelines.
- Order and interpret laboratory tests to monitor and enhance medication efficacy and safety.
- Report and document adverse drug events meticulously, adhering to best practices and regulatory standards.
- Provide comprehensive drug information and education, promoting informed decision-making among patients and providers.
- Mentor pharmacy students and residents, fostering the next generation of healthcare innovators.
- Adapt quickly to evolving healthcare demands, contributing to a culture of continuous improvement.
Minimum Qualifications
- PharmD or equivalent experience from an accredited pharmacy school.
- Completion of an ASHP-accredited PGY-1 Pharmacy Residency.
- Active license from the Texas Board of Pharmacy.
- Certification in Basic Life Support, First Aid, and CPR.
- Desirable: Board Certification as a Pharmacotherapy Specialist or Ambulatory Care Pharmacist.
- At least 2 years of community pharmacy and medication adherence counseling experience preferred.
- Experience in specialty areas such as HIV/AIDS, geriatrics, or pediatrics is advantageous.
- Proficiency in computer applications, including Windows, Microsoft Word, and Excel.
- Ability to manage multiple priorities and deadlines effectively.
- Excellent communication and customer service skills, with a strong sense of professional discretion.
- Desirable: In-depth knowledge of advanced pharmacy practices and evidence-based medicine.
