Job Overview

Join Legacy Community Health in a forward-thinking role as an AmeriCorps Member - Community Health Worker, where creativity meets impact in the vital realm of community health. In this position, you will be an integral part of our Transitions of Care team, ensuring seamless patient follow-up after hospital or emergency visits. This role is centered on providing crucial health education and screenings to elevate community health outcomes at our LMA location (2929 Allen Pkwy, Houston, TX 77019).

Key Responsibilities

  • Provide comprehensive health education and screenings, focusing on enhancing health outcomes and access to care.
  • Deliver relevant health education sessions tailored to community needs, covering topics like diabetes, maternal/child health, cancer, family planning, and HIV.
  • Conduct detailed health screenings, including assessments for diabetes, blood pressure, cholesterol, and sexual health.
  • Facilitate appointments for primary and preventative care at Legacy for individuals with identified health care needs.
  • Participate actively in health fairs, community meetings, and presentations to expand community outreach.
  • Connect community members and patients to primary and preventative healthcare services.
  • Link community members to eligibility services and educate them about comprehensive healthcare services offered by Legacy.
  • Represent the AmeriCorps Program by wearing the AmeriCorps logo daily.
  • Engage in volunteer or required disaster response and recovery activities as needed.
  • Collaborate with community partners to identify and execute service opportunities.
  • Provide translation and interpretation services when appropriate.

Minimum Qualifications

  • High School diploma or GED is necessary.
  • No prior experience is required.
  • Preferred experience with Electronic Health Record Systems (EHRS).
  • Strong communication and customer service skills.
  • Ability to multitask efficiently in a fast-paced environment.
  • Above average organizational and computer skills with a robust attention to detail.
  • Proficient in managing confidential and sensitive information securely.
  • Bilingual/Spanish fluency is considered advantageous.
