Key Responsibilities

  • Pioneer community health assessments utilizing advanced social determinants of health tools.
  • Establish and nurture relationships with refugee and immigrant communities to enhance healthcare navigation.
  • Conduct transformative home and site visits to identify service needs, providing game-changing information and referrals.
  • Document case records with precision, leveraging tech for seamless and timely management.
  • Champion Legacy service lines, forging pathways to establish Legacy as a premier medical home.
  • Lead as a peer trainer, fostering growth and capacity-building within the team.
  • Maintain and expand partnerships with Connect Community for synergistic impact.
  • Innovate community linkage to primary and preventive health services.
  • Empower community members with knowledge of comprehensive health services via Legacy.

Minimum Qualifications

  • High School diploma or GED is essential.
  • While experience is not mandatory, a passion for community engagement and health innovation is crucial.
  • Exceptional communication and customer service skills to revolutionize client interactions.
  • Astute multitasking abilities in a dynamic, fast-paced ecosystem.
  • Organizational prowess with above-average skills, enabling agile workflow management.
  • Proficient in technology with strong typing and computer skills, essential for digital transformation.
  • Bilingual capabilities, especially Spanish fluency, are highly advantageous.
  • Commitment to maintaining the confidentiality and security of sensitive information.
