About the Role

Embark on a transformative journey as an AmeriCorps Member, where your role as a Community Health Worker will bridge the gap between patients and imperative chronic disease management and prevention education services at Legacy Community Health in our LMA location (2929 Allen Pkwy, Houston, TX 77019). Key Responsibilities

  • Provide health education and screenings to enhance health outcomes and increase care access.
  • Deliver pertinent health education sessions targeting diseases like diabetes, cancer, and HIV.
  • Conduct comprehensive health screenings, including diabetes, blood pressure, and cholesterol.
  • Facilitate appointments for primary and preventative care at Legacy Health.
  • Engage in community outreach through health fairs and meetings.
  • Connect community members to essential health and eligibility services.
  • Educate about Legacy's extensive health services offerings.
  • Represent Legacy’s AmeriCorps Program professionally by wearing the AmeriCorps logo daily.
  • Assist in disaster response and recovery activities if needed.
  • Collaborate with community partners for service opportunity identification.
  • Perform additional duties as instructed that align with role responsibilities and accommodations.
  • Provide translation and interpretation services as needed.

Minimum Qualifications

  • High School diploma or GED.
  • No previous experience required, making it an excellent entry-level role.
  • Strong communication and customer service skills.
  • Effective multitasking in fast-paced settings.
  • Above average organizational skills and strong computer proficiency.
  • Capability of managing confidential and sensitive information with discretion.
  • Previous experience with Electronic Health Record Systems is advantageous.
  • Bilingual/Spanish fluency is preferred to enhance communication skills.
