Key Responsibilities
- Provide pharmaceutical care services through direct patient and provider interactions.
- Conduct chronic disease state medication management services via face-to-face or virtual care.
- Obtain comprehensive medical and medication histories to ensure safe and effective care.
- Reconcile patient medications during ambulatory care clinic visits.
- Perform physical assessments and triage patients as needed.
- Document encounters and interventions in the electronic medical record.
- Design, implement, and monitor therapeutic plans under your scope of practice.
- Initiate, modify, renew, refill, or discontinue medication therapy.
- Order labs and tests to determine the efficacy and toxicity of medication therapy.
- Document and report adverse drug events and medication allergies.
- Provide drug information and education to patients, families, and providers.
- Assess medication adherence during counseling sessions.
- Set priorities and work independently while collaborating with others.
- Serve as a preceptor to pharmacy students and residents.
- Perform other duties as assigned.
Minimum Qualifications
- Graduate of an accredited college/university of pharmacy (PharmD or B.S. with equivalent experience)
- Completion of ASHP accredited PGY-1 Pharmacy Residency.
- Valid license from the Board of Pharmacy of the State of Texas.
- Certification in Basic Life Support / First Aid / CPR.
- 2+ years of medication adherence counseling experience.
- Experience in HIV/AIDs, Geriatrics, and/or Pediatrics is preferred.
- Completion of ASHP accredited PGY-2 Pharmacy Residency in Ambulatory Care, Adult Medicine, Geriatric Medicine, or Pediatrics is preferred.
- Certification in immunization delivery or smoking cessation is preferred.
- Computer experience with Windows, Microsoft Word & Excel.
- Must be able to manage multiple deadlines and prioritize.
- Excellent communication skills and customer service skills.
