Patient Educator - Job Overview
Schedule: Monday-Friday (8AM-4:30PM or 8:30am-5PM)

Through health education, disease self-management, goal-setting, motivational interviewing, point-of-care testing, and care navigation, you will help patients build confidence and make progress toward their personal health goals. Your work will connect patients to essential Legacy services while strengthening prevention, wellness, and community healthcare.

Key Responsibilities
- Implement health education programming in one-on-one, co-visit, and group settings to increase the knowledge and self-efficacy of patients to prevent illness, manage health conditions, and improve overall wellness.
- Deliver education on core topics determined by clinic assignment; topics may change based on patient and clinic needs.
- Assist patients to set goals related to their health based on assessments of patient needs, including behavioral, social, and medical needs, readiness for health behavior change, and desired outcomes.
- Conduct point-of-care testing, such as blood glucose, HIV testing, and pregnancy testing, as well as health screenings and risk assessments.
- Guide patients in navigating the system of care in order to achieve health goals and access Legacy services.
- Document all patient services in the electronic health record and other data systems as required.
- Serve as a member of an interdisciplinary clinic and care team while promoting effective working relationships with internal and external stakeholders.
- Identify, develop, and maintain presentations, handouts, equipment, models, teaching tools, and other instructional materials that effectively convey information to patients and community members.
- Participate in the design and testing/evaluation of new health education curricula and teaching materials to support patient, clinic, and community needs.
- Meet assigned performance metrics, including those related to grant/contract requirements, if applicable.
- Participate in clinic, team, departmental, and agency meetings, trainings, and workshops as directed, including travel to different Legacy clinics.
- Take on other duties as assigned, contributing as a supportive and engaged member of the team.
Minimum Qualifications
- High School Diploma or equivalent
Required Years of Experience
- 3 - 5 years of relevant experience
Desirable Education and/or Experience
- Bachelor’s Degree in a related field with a minimum of 1 year of relevant experience preferred.
Required Knowledge, Skills, and/or Abilities
- Experience providing direct education to clients of diverse cultures and backgrounds in one-on-one and/or group settings.
- Experience in a healthcare setting.
- Experience with computers and computer programs.
- Ability to provide point-of-care testing within 3 months if/as required per site assignment.
- Ability to become phlebotomy trained within 3 months of hire if/as required per site assignment.
- Ability to demonstrate core competencies within 3 months of hire.
Desirable Knowledge, Skills, and/or Abilities
- Bilingual English/Spanish.
- Experience working in an electronic health record system.
- Experience working with people living with or at-risk for HIV.
- Experience working with sexual minorities and gender non-binary persons.
- Experience providing education about pregnancy and/or the post-partum period.
- Experience working with people who have chronic health conditions.
