Pharmacist - MTM/Specialty (Job Overview)

Key Responsibilities

  • Provide Medication Therapy Management (MTM) services, including personalized counseling, enrollment, and reassessment for specialty pharmacy patients.
  • Promote and enroll patients into the Specialty Pharmacy program to ensure they receive comprehensive medication support.
  • Perform regular reassessments (6-month and annual) to evaluate and optimize therapy plans for improved outcomes.
  • Conduct drug utilization reviews to identify potential duplications, interactions, and ensure safe, effective therapy.
  • Collaborate with patients and healthcare providers to address gaps in care through proactive outreach and medication profile reviews.
  • Deliver patient education on medication adherence, side effects, and proper usage to empower informed decision-making.
  • Ensure medication orders align with patient diagnoses, allergies, and weight/body surface area for precision care.
  • Stay up-to-date on new drugs, devices, and medical advancements to promote evidence-based practices.
  • Document and communicate adverse drug reactions to patients and providers for timely interventions.
  • Mentor pharmacy employees, students, and other healthcare professionals to foster a culture of learning and growth.
  • Demonstrate financial competence in managing grants, prescription assistance programs, and 340B compliance.
  • Exhibit professionalism, sensitivity, and compassion in all interactions with patients and team members.
  • Maintain clear and timely communication with supervisors and colleagues to address challenges and improve workflows.
  • Participate in continuous improvement initiatives to enhance patient care and operational efficiency.

Minimum Qualifications

  • Graduate of an accredited college/university of pharmacy (Pharm.D. or B.S. with equivalent experience).
  • Current Texas Pharmacist licensure in good standing (or eligible).
  • Certification in immunization delivery, smoking cessation, and naloxone (or eligible)
  • Computer experience, including Windows, Excel, and accounting software.
  • Must maintain BLS/CPR.
  • Spanish speaking preferred.
  • 3+ years of healthcare experience is required.
  • Post-graduate residency preferred.
  • 1+ years of community pharmacy experience preferred.
  • 1+ years of medication adherence counseling experience preferred.
  • 1+ years of Medicare market experience preferred.
  • Experience in HIV/AIDs, Geriatrics, and/or Pediatrics preferred.
  • Pleasant, professional, and articulate phone voice
  • Ability to multi-task and handle many incoming calls at once
  • Efficient and accurate data-entry skills
  • Demonstrates string computer skills. 
  • Bilingual in English/Spanish preferred.
