Key Responsibilities

  • Work with patients and families to ensure timely access to care and continuity of services.
  • Screen and assess individuals for social service needs, creating actionable care plans tailored to each patient.
  • Provide referrals and connect patients with medical, psychosocial, and essential community resources.
  • Conduct outreach for new patients, follow up on missed appointments, and advocate to close service gaps.
  • Educate and empower patients to actively participate in their care and advocate for themselves.
  • Monitor the effectiveness of services through periodic evaluations and quality assurance checks.
  • Maintain accurate and consistent documentation of patient interactions and care plans.
  • Participate in performance improvement and quality assurance initiatives to enhance patient care.
  • Obtain certification under Medicaid's Case Management for Children and Pregnant Women (CPW) Program and manage related caseloads.
  • Deliver direct patient care, including assessment, diagnosis, and treatment when appropriate.
  • Support the onboarding of new social service staff and mentor team members to foster their growth.
  • Engage in community coalition meetings to represent Legacy and advocate for patient needs.
  • Provide walk-in Behavioral Health/Social Work support as needed.
  • Travel between Legacy locations to meet patient and program needs.

Minimum Qualifications

  • MSW degree from a CSWE-accredited institution required.
  • Active licensure as defined by the Texas State Board of Social Worker Examiners.
  • BLS/CPR Certification.
  • Preferred experience working with children with special needs and pregnant women.
  • Knowledge of the National Association of Social Work Code of Ethics. Strong communication skills with diverse populations.
  • Proficiency in Microsoft Office suite.
  • Proven knowledge in community resources and public benefits landscape.
  • Ability to manage multiple deadlines and handle decision-making responsibilities.
  • Bilingual (English/Spanish) preferred.
