Position Summary
A Pharmacist Intern will work within a multi-disciplinary team to provide exceptional integrated care at Legacy Community Health under the supervision of a precepting pharmacist. Under a supervision of a licensed pharmacist, a Pharmacist Intern performs patient counseling, drug utilization reviews of patient profiles to prevent drug-drug interactions, drug-disease interactions, and duplicative therapies, and to increase medication adherence for the ages of clients served (0-adult).
Essential Duties & Functions
Under the supervision of a pharmacist, the pharmacist intern will:

Under a supervision of a licensed pharmacist, a Pharmacist Intern performs patient counseling, drug utilization reviews of patient profiles to prevent drug-drug interactions, drug-disease interactions, and duplicative therapies, and to increase medication adherence for the ages of clients served (0-adult).
Essential Duties & Functions
Under the supervision of a pharmacist, the pharmacist intern will:
- Perform basic dispensing functionalities such as data entry, preparation of medications, completion of sales, counseling, etc.
- Perform outreach, review, and enrollment of select patients into the specialty pharmacy program.
- Support patient outreach efforts for other on-going projects within the pharmacy as needed.
- Ensure the appropriateness of all medication orders, new prescriptions or refills, in reference to the client’s diagnosis, weight/body surface area, allergies, and drug interactions as evidenced by a review of the active medication profile, provider notes, and available pertinent laboratory information.
- Practice pharmacy in compliance with all governing regulatory authorities.
- Review patient profile to ensure appropriate drug selection and dosage.
- Conduct/Review Drug Utilization Reviews to check for drug duplication, interactions or allergies.
- Document adverse drug reactions and all relevant interactions with patient and healthcare provider.
- Provide accurate, adequate, drug information/education to patients, families, and providers on drug interactions, side effects, dosage, and storage of pharmaceuticals.
- Assess medication adherence during the process of medication counseling.
- Administer immunizations under protocol.
- Demonstrate the ability to set priorities, work independently and cooperatively with others within and outside the department.
- Present information (verbal and/or written) in a clear and concise manner.
- Display friendliness, compassion, sensitivity, kindness and appropriate manners in interactions with patients, physicians and other ancillary healthcare staff.
- Display open-mindedness and objectivity in relations with patients, physicians, and other ancillary healthcare staff.
- Perform other duties as assigned.
- Enrollment in an accredited college/university of pharmacy
- Certification in immunization delivery, smoking cessation, and naloxone (or eligible)
- Valid pharmacist intern license from the Board of Pharmacy of the State of Texas
- Must maintain BLS /CPR
- Experience in/knowledge of HIV/AIDs, Geriatrics, and/or Pediatrics
- Computer experience – Windows, Excel, and Accounting software.
- Must be able to manage multiple deadlines and prioritize
- Ability to speak Spanish
