Be at the forefront of driving healthy change in our communities with Legacy Community Health. As a Care Team Assistant (CTA), you’ll serve as a vital bridge of communication and information for patients, the call center, the care team, family/caregiver(s), and various community resources at our Deer Park location (3430 Center St, Deer Park, TX 77536).

Your role is designed to support the care team in offering quality and compassionate care, grounded in evidence-based practices and aligned with Patient Centered Medical Home (PCMH) requirements.

  • Act as a primary contact and informational resource for patients and healthcare partners.
  • Provide both clinical and clerical support to streamline care delivery processes.
  • Engage in proactive pre-appointment planning, medication management, and referrals.
  • Foster a collaborative team environment focused on patient-centered care.
  • Identify and address the needs of vulnerable population groups through additional services.

Key Responsibilities

  • Manage patient communication channels actively and empathetically.
  • Engage in comprehensive pre-visit planning to optimize care delivery.
  • Coordinate care, including medication management and referrals.
  • Promote and maintain effective working relationships within the care team.
  • Participate in required safety training and adhere to patient safety protocols.
  • Ensure compliance with LCHS's exposure control plans and safety standards.

Qualifications and Skills

  • High-school level education or equivalency required.
  • Medical Assistant Certification or equivalent experience, or at least 2 years of clinical experience in an inpatient or outpatient setting.
  • Must have and maintain BLS/CPR certification.
  • At least 2 years of experience in an ambulatory clinical setting preferred.
