Join Legacy Community Health in an innovative role that touches lives and transforms communities. As an AmeriCorps Community Health Worker focusing on Early Childhood Development, you'll be at the forefront of promoting accessible healthcare and strengthening community ties in our LMA location (2929 Allen Pkwy, Houston, TX 77019).
- Engage with the community to promote Legacy Community Health's accessible healthcare services at events.
- Forge durable partnerships with local organizations to enhance healthcare accessibility and community connectedness.
- Assist patients with follow-up services to maintain enduring healthcare relationships and secure ongoing support.

Key Responsibilities
- Educate community members about available healthcare programs and services at Legacy Community Health.
- Develop and nurture relationships with local community organizations to bridge care gaps and enhance community connections.
- Assist patients in overcoming financial barriers to healthcare through community outreach and enrollment in programs like Medicaid and CHIP.
- Facilitate connections between patients and primary, preventative healthcare appointments.
- Organize outreach events to promote healthcare access in underserved areas.
- Provide follow-up services to ensure patients' healthcare needs are continuously met.
- Perform additional assigned duties, connecting community members to healthcare services, eligibility services, and more.
Minimum Qualifications
- High School diploma or GED is required.
- No prior experience is required, making this an excellent entry point into community health.
- Experience with Electronic Health Records System (EHRS) is preferred.
- Exceptional communication and customer service skills are essential.
- Capability to multitask in a dynamic, fast-paced setting.
- Above-average organizational skills to manage multiple community health initiatives.
- Strong typing and computer proficiency.
- Bilingual candidates with Spanish fluency are highly preferred to better serve diverse communities.
