As an AmeriCorps Prevention Coordinator, you will play a crucial role in guiding patients through complex healthcare systems, both within Legacy and with external providers. Your focus will be on breaking down barriers to cancer prevention and care by emphasizing the importance of screenings, vaccinations, and comprehensive healthcare services. This role will be based at our LMA location (2929 Allen Pkwy, Houston, TX 77019).

Key Responsibilities

  • Motivate and encourage patients through phone contact to complete preventive cancer screenings.
  • Facilitate timely completion of screenings for colorectal, breast, and cervical cancer, as well as HPV vaccinations.
  • Provide appointment reminders and follow-up communications with patients.
  • Coordinate and confirm medical appointments, collaborating with Legacy and community providers.
  • Enter detailed notes into Electronic Health Records after each client interaction.
  • Collaborate with the healthcare team to provide cohesive and integrated healthcare services.
  • Maintain project communication and updates with Site Directors and Contact Centers.
  • Track and report on BI process and outcome data for key projects.
  • Compile monthly statistics and required reports with precision.
  • Foster positive relationships with Providers, Site Directors, Managers, and Care Teams.
  • Maintain patient confidentiality as per HIPAA regulations, demonstrating discretion with sensitive information.
  • Exhibit professionalism, composure under pressure, and accountability in all work commitments.
  • Perform other duties as assigned, supporting a diverse and inclusive workplace.

Minimum Qualifications

  • High School diploma or GED required.
  • No prior experience necessary, but a passion for community health is essential.
  • Exceptional communication and customer service skills.
  • Ability to effectively multitask in a dynamic, fast-paced environment.
  • Strong organizational skills with a detail-oriented mindset.
  • Proficient in typing and computer operations.
  • Capable of managing multiple deadlines with efficiency.
  • Bilingual/Spanish fluency is preferred, enhancing communication with a broader patient base.
