About the Position

Welcome to the dynamic world of Legacy Community Health, where we are pioneering a new approach to community healthcare! As a Care Team Assistant (CTA) at our Legacy Santa Clara Clinic, you'll be at the heart of our mission to drive healthy change. Positioned as the central communication hub, the CTA is a vital link for patients, our call center, caregivers, and community resources, ensuring seamless interaction and support. Leveraging both clinical and clerical skills, the CTA uplifts the care team’s capabilities, enabling the delivery of evidence-based, compassionate care aligned with Patient Centered Medical Home (PCMH) standards. Your role extends beyond just administrative support; you'll assist in critical aspects such as patient communication, medication management, and proactive care planning, all while identifying and supporting vulnerable populations.

Key Responsibilities

  • Patient Communication: Serve as the frontline responder for patient calls, coordinating effectively with the care team to address clinical inquiries, and ensure timely communication of lab results and medical instructions.
  • Pre-Visit Planning: Facilitate comprehensive pre-visit preparations by ensuring that test results and care records are ready prior to patient appointments; collaborate with healthcare partners to overcome potential barriers.
  • Care Coordination: Support seamless transitions of care, manage the Care Coordination Desktop, and connect patients with essential ancillary services, promoting holistic health management.
  • Medication Management: Assist in managing medication refills and prior-authorizations, working closely with providers to ensure patients’ medication needs are met efficiently.
  • Forms Handling: Initiate the accurate completion and processing of critical forms, including disability applications and health order requests, ensuring prompt attention for urgent needs.
  • Referrals: Collaborate with Referral Coordination staff to finalize referrals and deliver comprehensive clinical summaries to external service providers.

Qualifications

  • High School Diploma or equivalent
  • Medical Assistant Certification or equivalent experience
  • Alternatively, 2 years of relevant experience in an inpatient or outpatient setting
  • 2 years of experience in an ambulatory clinical setting
  • Bilingual in English and Spanish preferred
