Key Responsibilities

  • Receive and effectively manage inbound patient calls following best practice guidelines.
  • Gather, prioritize, and document necessary data to ensure thorough patient care integration.
  • Address healthcare concerns, providing clinical advice and education as per set protocols.
  • Assist in coordinating comprehensive patient care and referrals as necessary.
  • Employ approved protocols to provide informed telephone advice and patient guidance.
  • Document all patient interactions meticulously in the medical record system.
  • Uphold patient privacy and confidentiality consistently.
  • Deliver non-judgemental, culturally informed care in a respectful manner.
  • Identify ongoing patient care trends and concerns for proactive management.
  • Demonstrate readiness in activating backup support to ensure timely patient responses.
  • Perform other duties as assigned to contribute to team success.

Minimum Qualifications

  • Active LVN or LPN license in the State of Texas, with an unrestricted license or valid compact state licensure.
  • Current BLS (CPR & AED) certification.
  • A minimum of 1 year relevant experience, preferably in a primary care setting.
  • Strong clinical, interpersonal, and communication skills, both written and verbal.
  • Fluency in Spanish is strongly preferred and advantageous.
  • Proven ability to manage multiple deadlines and prioritize tasks effectively.
  • Experience within a call center environment is preferred.
  • Dedicated to non-discriminatory patient care and confidentiality.
