Key Responsibilities
- Implement innovative health education programming to amplify patient knowledge and self-efficacy
- Conduct pregnancy testing, health screenings, and risk assessments using the latest methodologies
- Empower patients in health goal-setting and adept navigation of care systems
- Document patient services with precision in electronic health records
- Expertly manage dynamic patient schedules
- Collaborate on interdisciplinary care teams for comprehensive health solutions
- Maintain robust referral processes and up-to-date resource lists
- Develop and innovate educational materials for maximum impact
- Participate in the development of groundbreaking health education curricula
- Consistently meet performance metrics and engage in strategic meetings

Minimum Qualifications
- High School Diploma or equivalent with 3 - 5 years of relevant experience
- Preferred: Bachelor's Degree in a related field with at least 1 year of experience
- Proven experience in direct education within diverse cultural environments
- Proficiency in healthcare, digital tools, and innovative patient engagement techniques
- Ability to conduct health screenings and demonstrate Prenatal Navigator competencies
- Bilingual English/Spanish is highly desirable
- Experience with electronic health records and patient care in specialized areas such as HIV, gender non-binary, or chronic conditions
- Resilience in managing physical and mental demands
- Agility in adapting to varied work environments and technological advancements
