Key Responsibilities

• Implement innovative health education programming to amplify patient knowledge and self-efficacy
• Conduct pregnancy testing, health screenings, and risk assessments using the latest methodologies
• Empower patients in health goal-setting and adept navigation of care systems
• Document patient services with precision in electronic health records
• Expertly manage dynamic patient schedules
• Collaborate on interdisciplinary care teams for comprehensive health solutions
• Maintain robust referral processes and up-to-date resource lists
• Develop and innovate educational materials for maximum impact
• Participate in the development of groundbreaking health education curricula
• Consistently meet performance metrics and engage in strategic meetings
• Provide cutting-edge training and sustain professional acumen

Minimum Qualifications

• High School Diploma or equivalent with 3 - 5 years of relevant experience
• Preferred: Bachelor's Degree in a related field with at least 1 year of experience
• Proven experience in direct education within diverse cultural environments
• Proficiency in healthcare, digital tools, and innovative patient engagement techniques
• Ability to conduct health screenings and demonstrate Prenatal Navigator competencies
• Bilingual English/Spanish is highly desirable
• Experience with electronic health records and patient care in specialized areas such as HIV, gender non-binary, or chronic conditions
• Resilience in managing physical and mental demands
• Agility in adapting to varied work environments and technological advancements
