Ryan White Service Linkage Worker - Job Overview

Schedule: Monday-Friday (8AM-5PM or 9AM-6PM)

Key Responsibilities

  • Assist patients in navigating service delivery systems and agency procedures.
  • Collaborate with medical case managers and clinic staff to implement patient care plans.
  • Monitor patients' progress by reviewing attendance at HIV primary care appointments.
  • Ensure referral appointments are made and kept by following up with patients.
  • Assist patients in obtaining necessary services such as transportation or translation.
  • Conduct assessments to determine medical and psychosocial history and current needs.
  • Create and manage care plans for referred patients and families.
  • Provide information, referrals, and assistance with linkage to necessary services.
  • Document all services provided in the EHRS client record within 72 hours.
  • Advocate on behalf of clients to decrease service gaps and eliminate barriers.
  • Connect with Legacy staff and providers for effective system navigation.
  • Collaborate with community-based agencies to support linkage to social supports.

Minimum Qualifications

  • Bachelor's Degree in social or behavioral sciences, or High School Diploma/GED with five years of relevant work experience.
  • Minimum of 1-year paid work experience with Persons Living with HIV/AIDS (PLWH).
  • Familiarity with Ryan White grants and services is preferred.
  • Experience with EPIC systems is a plus.
  • Bilingual candidates, especially those fluent in Spanish, are encouraged to apply.
