Ryan White Service Linkage Worker - Job Overview
Schedule: Monday-Friday (8AM-5PM or 9AM-6PM)

Key Responsibilities
- Assist patients in navigating service delivery systems and agency procedures.
- Collaborate with medical case managers and clinic staff to implement patient care plans.
- Monitor patients' progress by reviewing attendance at HIV primary care appointments.
- Ensure referral appointments are made and kept by following up with patients.
- Assist patients in obtaining necessary services such as transportation or translation.
- Conduct assessments to determine medical and psychosocial history and current needs.
- Create and manage care plans for referred patients and families.
- Provide information, referrals, and assistance with linkage to necessary services.
- Document all services provided in the EHRS client record within 72 hours.
- Advocate on behalf of clients to decrease service gaps and eliminate barriers.
- Connect with Legacy staff and providers for effective system navigation.
- Collaborate with community-based agencies to support linkage to social supports.
Minimum Qualifications
- Bachelor's Degree in social or behavioral sciences, or High School Diploma/GED with five years of relevant work experience.
- Minimum of 1-year paid work experience with Persons Living with HIV/AIDS (PLWH).
- Familiarity with Ryan White grants and services is preferred.
- Experience with EPIC systems is a plus.
- Bilingual candidates, especially those fluent in Spanish, are encouraged to apply.
