Care Team Assistant - Job Overview

Schedule: 8AM-5PM

Key Responsibilities

  • Patient Communication: Serve as the first point of contact for patient inquiries, working closely with onsite nursing and following protocol-based triage guidelines.
  • Route clinical inquiries to providers via the electronic health record system (EHRS), ensuring timely responses.
  • Deliver lab results and other important information to patients through calls or template-based letters.
  • Reach out to medically vulnerable patients post-visit, ensuring they receive necessary follow-up care.
  • Pre-Visit Planning: Review daily patient schedules and assist in preparing for upcoming visits.
  • Arrange interpretation services and participate in daily team huddles to ensure smooth operations.
  • Care Coordination: Facilitate seamless transitions of care by obtaining discharge records and specialty notes.
  • Manage the Care Coordination Desktop and assist patients with accessing ancillary support services.
  • Medication Management: Help with medication refills and prior-authorizations, ensuring patients receive necessary medications promptly.
  • Forms and Referrals: Initiate and manage the completion of forms, assist with referrals, and prepare clinical summaries for external consultants.

Minimum Qualifications

  • High-school level education or equivalency.
  • Medical Assistant Certification or its equivalent, or at least 2 years clinical experience in an inpatient or outpatient setting.
  • Must maintain BLS/CPR.
  • At least 2 years of experience in an ambulatory clinical setting preferred.
  • Spanish fluency
  • Demonstrated competency with an EHRS system.
  • Excellent communication skills.
  • Excellent customer service skills.
  • Ability to multi-task in a fast paced environment.
  • Above average organizational skills.
  • Strong typing and computer skills.
