Bilingual Care Team Assistant - Job Overview

Schedule: Monday-Friday (8:30AM-5PM)

Key Responsibilities

  • Patient Communication: Work with onsite nursing or follow protocol-based triage to address patient phone calls and route clinical inquiries to providers. Return calls and communicate results as directed.
  • Pre-Visit Planning: Review patient schedules daily, ensure lab results and consultation reports are available, and gather necessary information. Assist in arranging interpretation services and participate in daily huddles.
  • Care Coordination: Manage transitions of care, support access to ancillary services, and maintain the Care Coordination Desktop.
  • Medication Management: Assist with medication refills and prior-authorizations.
  • Forms: Initiate and complete forms for review and final signature by the provider. Address urgent requests promptly.
  • Referrals: Complete referrals, deliver clinical summaries for consultants, and ensure relevant documents are included in patient records.
  • Safety and Compliance: Promote effective working relations to facilitate goal achievement and adhere to safety training and equipment operation.

Minimum Qualifications

  • High-school level education or equivalency
  • Medical Assistant Certification or its equivalent, or at least 2 years clinical experience in an inpatient or outpatient setting
  • Must maintain BLS /CPR
  • At least 2 years of experience in an ambulatory clinical setting preferred 
  • Spanish fluency
  • Demonstrated competency with an EHRS system
  • Excellent communication skills.
  • Excellent customer service skills.
  • Ability to multi task in a fast paced environment.
  • Above average organizational skills.
  • Strong typing and computer skills. 
  • This job requires access to confidential and sensitive information, requiring ongoing discretion and secure information management
  • Must be able to manage multiple deadline and decision making responsibilities.
