Key Responsibilities

  • Provide individualized care for specialty patients through clinical assessments and medication therapy management.
  • Conduct drug utilization reviews and address medication adherence to improve health outcomes.
  • Evaluate patient medication records to make therapeutic recommendations and close care gaps.
  • Offer medication counseling and disease state education to patients.
  • Ensure proper use of medications by reviewing patient profiles, including drug selection and dosing.
  • Comply with regulatory authorities while practicing pharmacy.
  • Document and address adverse drug reactions and interactions.
  • Engage in continuous education on new drugs and medical literature.
  • Provide drug information and education to patients and healthcare providers.
  • Collaborate with interdisciplinary teams and manage financial assistance programs.
  • Understand and manage the 340B program, inventory, and special billing conditions.
  • Participate in training and provide feedback for process improvements.
  • Check all communication methods, including email, regularly and keep the supervisor informed of issues.
  • Perform other duties as assigned.

Minimum Qualifications

  • Graduate of an accredited pharmacy school (Pharm.D. or B.S. with equivalent experience).
  • Current Texas Pharmacist licensure in good standing (or eligibility required).
  • Certification in Immunization Delivery (or eligibility required).
  • Certification in Intravenous Sterile Compounding (or eligibility required).
  • Proficiency in Microsoft Windows, Excel, PowerPoint, SharePoint, and accounting software.
  • Maintain BLS/CPR certification.
  • Spanish speaking preferred for patient communication.
  • A minimum of 3 years of healthcare experience is required.
