Key Responsibilities
- Serve as the vital link between Legacy clients, providers, and pharmacies to ensure swift fulfillment of medication needs.
- Efficiently monitor and manage incoming requests for prescription renewals through electronic systems, pharmacies, and direct patient contact, ensuring prompt responses within 48 hours.
- Evaluate renewal requests to determine compliance with established guidelines, consulting pharmacists and providers as needed to maintain protocol integrity.
- Comprehend and execute follow-up plans as outlined by providers to maintain optimal patient care.
- Identify and proactively address patients at risk of losing access to essential prescriptions, including scheduling appointments, providing refills, and facilitating necessary communications and lab orders.
- Handle incoming requests for medication prior authorizations, submitting these on behalf of providers and patients, while accurately maintaining documentation and status updates in both the Legacy EMR and Legacy Pharmacy systems.
- Maintain exemplary records of all activities, contributing to the seamless operation of Legacy Community Health's pharmaceutical services.
- Embrace and execute additional tasks as assigned, contributing to the health ecosystem's continual improvement.
Qualifications
- High School Diploma or equivalent is required.
- Certification as a Pharmacy Technician or LVN preferred.
- Proficient in using electronic medical records and pharmacy systems.
- Strong communication and problem-solving skills.
- A proactive approach to managing patient-care logistics and enhancing healthcare delivery.
- Demonstrated ability to work effectively in a collaborative, team-oriented environment.
