Care Team Assistant - Job Overview

Key Responsibilities

  • Dynamic Patient Communication: Be the leading voice in patient outreach, delivering critical information and fostering an informed patient community.
  • Data-Driven Pre-Visit Planning: Utilize electronic health record systems to optimize patient schedules and prepare pertinent information for seamless care delivery.
  • Innovative Care Coordination: Facilitate patient transitions with advanced coordination strategies, ensuring comprehensive access to necessary services.
  • Efficient Medication Management: Oversee medication refills and prior-authorizations with precision and tech-savvy processes.
  • Robust Forms and Referrals Management: Streamline form processing and referrals to enhance operational efficiency.
  • Proactive Safety and Compliance: Lead in maintaining a safe and compliant healthcare environment through continuous training and proactive measures.

Minimum Qualifications

  • Educational Foundation: A high-school diploma or equivalent is essential.
  • Professional Certifications: Medical Assistant Certification or minimum two years of clinical experience, with an active BLS/CPR certification.
  • Practical Experience: Two years of experience in an ambulatory clinical setting is highly desirable.
  • Advanced Skills: Proficiency in electronic health record systems, stellar communication capabilities, adept multitasking, superb organizational abilities, and technology literacy.
  • Language Proficiency: Spanish fluency is a significant advantage.
  • Confidentiality and Discretion: Maintain the highest standards of information confidentiality and integrity.
