About the Position
Outreach workers focus on locating clients who are on the cusp of falling out of care, for reengagement back into care. The Outreach Worker (OW) provides field-based services to clients based on criteria identified by each agency. These services include the provision of information, referrals and assistance with linkage to medical, mental health, substance abuse and psychosocial services as needed and advocating on behalf of clients to decrease service gaps and remove barriers to services.
Key Responsibilities
- Re-engage persons with previously diagnosed HIV, out-of-care to HIV medical care and ART
- Engages in patient interactions via telephone and in-person to identify, advocate, and meet client needs to ensure linkage and engagement to HIV care.
- Adapts work methods to specifically accommodate the population ' specific care needs (physical, psychosocial, cultural, age /developmental) of each patient served.
- Responsible for establishing trusting relationships with community partners, patients and their families, and provide general support and encouragement
- Assists patients to obtain or arrange for services such as transportation or language/translation services to eliminate possible barriers to care
- Conduct brief interventions with patients who present with low/intermittent needs such as eligibility renewals, ADAP application renewals, bus pass renewals, or information about a service.
- Provide information, referrals, and assistance with linkage to medical, psychosocial, and basic needs services
- Document service provided in EHRS client record within 72 hours
- Advocate for and on behalf of clients and families to decrease service gaps and remove barriers to services.
- Connect with relevant Legacy staff and providers to assist with system navigation, including Eligibility, Patient Assistance, Behavioral Health, Health Navigation, etc.
- Collaborate with community-based agencies to support linkage to social supports within the larger community
- Monitor the efficacy and quality of services through periodic re-evaluation with peers and supervisor.
- Participates in Legacy’s Performance Improvement and Quality Assurance Program.
- Demonstrates respect and regard for the dignity of all patients, families, visitors and fellow employees to insure a professional, responsible and courteous environment.

Our roots began in 1981 as the Montrose Clinic, with specialization in HIV education, testing, and treatment. Since then, the agency has expanded to >50 clinics in Houston, one in Baytown, two in Beaumont, and one in Deer Park with extensive services that include: Adult primary care, HIV/AIDS care, pediatrics, OB/GYN and maternity, dental, vision and behavioral health. Requirements
High School Diploma or GED required
Six months of working with or volunteering with PLWH (Persons Living with HIV/AIDS) required
Valid Driver License as position will require driving duties, required
Knowledge of Ryan White grants and services preferred
Experience in conducting outreach to engage individuals in HIV care preferred
EPIC experience preferred
Bilingual / Spanish fluency preferred
