Working at our clinic based at 1415 California St., Houston, TX 77006, you will be at the forefront of community-driven healthcare, connecting with individuals, providing vital health screenings, and paving the way for accessible healthcare services.

  • Engage directly with diverse communities to deliver preventive health screenings and education.
  • Participate in an environment that values creativity, teamwork, and growth with opportunities such as loan repayment programs.
  • Leverage technology and creativity to overcome barriers in healthcare accessibility.
  • Collaborate with a motivated team committed to enhancing public health outcomes.
  • Embrace a mission-driven approach to improve community health through innovative and forward-thinking strategies.

Key Responsibilities

  • Assist Legacy patients and community members in identifying and addressing barriers to healthcare services.
  • Provide health education and screenings to increase health outcomes and access to care.
  • Engage actively in health fairs, community meetings, and presentations to widen community outreach.
  • Conduct community health needs assessments using nationally recognized screening tools like PRAPARE.
  • Utilize a database to connect individuals with resources addressing social determinants of health.
  • Coordinate and support patients in accessing community resources and services.
  • Facilitate connections to primary and preventative health care services.
  • Educate the community about all health services provided by Legacy.
  • Represent Legacy’s AmeriCorps Program by wearing the AmeriCorps logo daily.
  • Participate on a volunteer basis or as required in disaster response and recovery activities.
  • Engage with community partners to identify service opportunities aligning with program goals.
  • Provide translation and interpretation services as needed.

Minimum Qualifications

  • High School Diploma or equivalent.
  • Passion for community outreach and healthcare services.
  • Strong communication skills and ability to engage with diverse community members.
