Legacy Community Health is pioneering a future where pharmacy services transcend traditional boundaries to meet real community needs. As an AmeriCorps Pharmacy Advocate, you'll be instrumental in addressing social determinants of health, helping increase patient knowledge and access to vital pharmacy services. This role is pivotal in connecting patients with affordable medications, promoting health literacy, and transforming health outcomes for individuals and families within the community. AmeriCorps is a Federal Program that connects Americans with Community Services in Health, Public Safety, etc.

Must commit to completing the 1700-hour requirement by August 31, 2026. This is a Temporary Position; Eligible for renewal for future AmeriCorps sessions.

Core Responsibilities

  • Utilize health information technology, including electronic health records (EHRs) and pharmacy management systems, to streamline workflows and enhance communication.
  • Provide technical support for medication counseling, refill requests, and medication delivery services.
  • Educate patients on the use of self-monitoring devices, enhancing their ability to manage health conditions at home.
  • Collaborate with healthcare providers to identify patients at risk of non-adherence and offer personalized support.
  • Increase patient awareness of medication and insurance assistance programs, guiding them through the application process.
  • Support pharmacy's preventative health initiatives through patient education on immunization, smoking cessation, and weight loss.
  • Organize brainstorming sessions to generate creative solutions for inventory and logistical challenges.
  • Participate in quality improvement initiatives and educational experiences to enhance pharmacy services.
  • Foster relationships with local healthcare providers and community leaders to maximize impact and share best practices.
  • Engage with the Legacy Community Health Corps, connect community members to primary and preventive healthcare services, and educate on all services offered by Legacy.
  • Represent Legacy's AmeriCorps Program and be prepared to assist in disaster response and recovery when needed.

Required Qualifications

  • High School diploma or GED.
  • Experience with EHRs preferred.
  • Exceptional communication and customer service skills.
  • Ability to multitask in a dynamic environment.
  • Above average organizational skills and strong typing and computer skills.
  • Discretion in managing confidential and sensitive information.
